La película narra la historia de Hedwig, vocalista transexual de la banda punk "The Angry Inch", mientras se encuentra en el proceso de búsqueda de su otra mitad, y muestra como, en el camino, y sin proponérselo, se encuentra a sí mismo.
Canciones cargadas de glam y punk rock, con facilidad para encontrar melodias efectivas y que han cautivado a todos, y que la mayoria termina tarareando.
